Adult Care Practitioner – Drive Independence & Safeguards

We Do Social Work in Telford & Wrekin is looking for an experienced Adult Practitioner to join its supportive adult social care team. This hybrid role allows a mix of office and home-based working, enabling you to make a meaningful difference by supporting adults in maintaining their independence.

You will complete various assessments, develop support plans, and work collaboratively with health professionals and families. An understanding of safeguarding, care planning, and strong assessment skills are essential for this role.
