As an Advanced Practice Provider Home InstaCare, you will work with a team of healthcare professionals committed to providing quality care and excellent patient experience. In this role, you will provide Urgent / InstaCare services to patients in their home.
+ This position includes a **$5,000 sign-on bonus** , up to **$7,500 in relocation** assistancewhen applicable, a competitive salary, andan **$18,000.00 Annual Quality Incentive**
+ Your home base will be Clinic Wynn – this role isprimarilyamobile practicedelivering care to the patient in their homesettingvia an Intermountain car/vehicle
+ Weekly schedule involves 3 x 12 hour days/ week
