Job Description

Functions as patient facing coordinator for the Northwell MAID (Medical Aid in Dying Program). Provides strategic advice and guidance to Social Work department. Provides professional input to ensure completion of departmental goals, compatible with overall needs & policies. Participates in the training and development of Social Workers.

Job Responsibility

+ Serves as the first point of entry for all MAID-related inquiries from patients, families, hospital staff, providers, and external stakeholders.
+ Conducts detailed psychosocial assessments and oversees appointments and eligibility verification
+ Collaborates closely with patients and their families to address all practical and logistical considerations regarding MAID
+ Provides supportive counseling and appropriate referrals to patients and their families throughout the MAID process.
