FlexStaff is seeking a Registered Nurse (RN) to work full-time as a Nurse Case Manager for an Assisted Living facility in Queens, NY.

In this role, you'll lead clinical assessments, coordinate individualized care plans, and serve as a critical liaison among residents, families, providers, and staff to promote safety, well-being, and independence.

Schedule: Monday - Thursday; 7:00AM - 5:00PM, day shift, with occasional early evening or weekend coverage for urgent needs.

_Salary 105,000-115,000 annually. Sign-on bonus $20,000._

Qualifications

+ Current, unencumbered New York State Registered Nurse (RN) license required.
+ 2+ years of clinical experience in assisted living, long-term care, home care, case management, or geriatric settings (ALP experience a plus).
