Behavioral Health Care Coordinator (BHCC) – PT, Weekends

Essential Duties & Responsibilities Accountabilities Lead daily programming activities, with the support of the Clinical Supervisor, to enhance the wellbeing and independent living skills of the residents in the program.
Accurately and thoroughly document interactions and efforts provided to the clients in our Electronic Health Record System (EHR).
Document any changes in residents behavior, mood, eating habits or medical concerns in the Electronic Health Record System (EHR).
Notify the Clinical Supervisor if condition is severe and needing further intervention.
Dispense or administer medications, as ordered by a physician, and document in the EHR system.
Use strength-based interventions to enhance a residents coping skills Teach residents self-care skills, such as proper hygiene and grooming.
Help maintain the cleanliness and safety of the facility by active participation and modeling.
