Care Coordinator (Community Behavioral Health Center)

Description of Position

We are seeking a full-time Care Coordinator to join our CBHC team in providing assistance to our clients with mental health struggles. The ideal candidate is interested in providing a compassionate and full continuum of care to clients under the umbrella of the CBHC. You will be part of a multidisciplinary team of addressing the social, emotional, psychiatric, and medical needs of the clients we serve; this includes clients across the lifespan with a variety of presentations, including mental health and substance use concerns. Important qualities to be successful in this role include being self-motivated, good personal boundaries, and a belief in the principles of hope and recovery from mental illness.
