We are currently looking for multiple full-time Care Delivery Manages with their RN License to come on-board with one of our Healthcare Clients in Queens & Westchester. The ideal candidate will have 5+ years of experience as a Registered Nurse, 2+ years of Managerial Experience, and the minimum of 1 year of experience working within a certified home health agency. These positions will be overseeing the Community Health Nurses that are working across the 5 Boroughs of NYC providing care within geriatrics. 

JOB RESPONSIBILITIES:
Review and ensure effective development and execution of personalized care plans that result in improved satisfaction & outcomes for all participants.
