Care Management Assistant

**Job Description:**

This is an exciting opportunity to join the Primary Children’s Outpatient Specialty team! The primary focus of this role will be to assess families for social drivers of health needs and then connect them with resources. This position supplements the work of the social work care manager in the holistic care of the pediatric patient. This role is on-site where you will meet with patients and family during clinic visits and by phone.
