Care Manager Assistant

**Job Description:**

This is an exciting opportunity to join our outpatient social work care management team in a new position! In this role you will work with families of pediatric patients seen in the at Primary Children's in the Child Development Program (CDP). As Care Manager Assistant you will assess for needs that patient and family with a focus on getting them to connected to resources such as insurance, housing, transportation, and behavioral health supports. This role will be on-site with support provided primarily by phone.
