Care Partner - Medical Assistant

**The Care Partner-Medical Assistant responsibilities include:**

+ Works collaboratively with the Licensed Practical Nurse (LPN) Care Partner to review patient chart data, identify open Healthcare Effectiveness Data and information Set (HEDIS) gaps and facilitates gap closure.
+ Partners with BayCare Medical Group (BMG) physicians and office medical staff to ensure gaps are addressed and closed.
