Care Partner, Support at Home

About the role
This is a full time position, fixed term for 12 months and can be located in Queanbeyan or Tuggeranong.
 
The Care Partner plays a central role in supporting older people receiving Support at Home services to live well, independently and safely at home. The role is responsible for building trusted relationships with older people, families and carers, understanding what matters most to each person, and co-designing support plans that reflect individual goals, preferences and needs.
