Case Manager for Prenatal Outreach Team

Sanctum Care Group is hiring a Case Manager for the Prenatal Outreach Resource Team in Prince Albert. This full-time role supports expectant mothers through comprehensive case management services including mental health and addiction support.
As a member of the PORT, you will coordinate psychosocial and health services for at-risk expectant women. Your commitment to trauma-informed, compassionate care is essential as you engage with clients and community partners. This position requires strong collaboration skills and the ability to work independently in a community-based environment.
Key Responsibilities:
• Provide case management to prenatal and postnatal women
• Maintain accurate charting and documentation

Requirements:
