Job Summary The Case Manager provides direct support and service coordination to individuals and/or families. This role uses a client-centered, strengths-based approach to assess needs, develop service plans, and support clients in achieving their goals. The Case Manager collaborates with internal teams and external partners to ensure effective service delivery. Job Responsibilities Conduct client assessments to identify needs, strengths, and goals.
