Description Kindred Hospital Baldwin Park helps patients return to the lives they love. At our 91‐bed hospital, our physician‐led care teams provide critically ill patients with the specialized acute care and rehabilitation they need, for the time they need it – empowering them to take the next step in their recovery journey. We also offer an eight‐bed ICU and three negative‐pressure rooms for treating particularly serious conditions. We are located near the I‐605 and I‐10 intersection in Baldwin Park with a number of shops and restaurants nearby for added convenience. Job Summary Coordinates and facilitates the care of the patient population through effective collaboration and communication with the Interdisciplinary Care Transitions (ICT) team members.
