The role. Reporting to the Rehab Program Director and Facility Case Management Director, this position guides patients and families through every step of their care journey — from program orientation and explanation of Medicare and insurance benefits to discharge criteria and patient rights. The Case Manager conducts psychosocial assessments and discharge planning evaluations, documenting findings and communicating needs across the interdisciplinary care team.
