JOB DESCRIPTION Manage casework processes from admission to discharge, including assessments, care planning and documentation. Complete required assessments on residents and families using appropriate tools to inform intervention plans. Assess residents' needs, strengths and risks develop and review Individualized Care Plans (ICP). Provide counselling and interventions for residents and families, including planning and executing counselling sessions.
