**Job Description :**

GENERAL OVERVIEW:

Assesses patient need for diabetes education. Provides individualized diabetes self-management education/training based on assessment. Plans and conducts continuing education programs for professional staff and the community.

ESSENTIAL RESPONSIBILITIES:

+ Collects DSME/T program participant assessment data, using a recognized certification database/program in a collaborative and ongoing manner.
+ Develops educational goals, learning objectives and a plan for educational content and teaching methods with program participants.
+ Collaboratively develops an individualized follow-up plan with each program participant. Evaluates effectiveness of educational services provided by measuring attainment of learning objectives.
