Clinical Care Coordinator

**Overview**

**Scope of Responsibilities:** Provide day-to-day support to clinical team and patient management program. Interact with health care professionals and patients to support clinical programs and initiatives.

**Job Summary:** Responsible for all aspects related to collecting, entering, and maintaining medical information within our electronic medical record platform. Participate in the collection and analysis of data for patient management and clinical programs. Responsible to ensure the highest quality healthcare delivery while complying with accreditation standards.

+ Responsible for onboarding new and existing patients to our patient management program. Ensuring compliance with all accreditations and manufacturer programs and protocols
+ Assist leadership by supporting compliance with accreditation standards organizations such as URAC, ACHC, and NABP
