SUMMARY


The Transitions of Care Clinical Staff Pharmacist – Transplant is critical in ensuring patients are prepared to be transitioned to different levels of care and have the information necessary to take their medications safely, appropriately, and consistently. The pharmacist will review the medications prescribed for appropriate dosing, frequency, and indications and address any discrepancies or recommendations with the patient’s primary team or prescriber. The pharmacist will provide counseling on their medication regimen and appropriate resources to educate the patient about medication instructions, uses, side effects, and additional medication therapy management considerations. The pharmacist may also participate in specialty consults such as anticoagulation counseling, heart failure counseling, outpatient parenteral antimicrobial therapy and home infusion review, solid organ transplant and left ventricular assist device (LVAD) education and pill box fi...
