Summary :

Serve as a bridge between rural residents of Cumberland County and clinical health care services. Identify community health needs, engage patients through mobile outreach, conduct chronic disease assessments, and provide chronic disease health and wellness education.

Primary Duties/Responsibilities :
+ Work with the Project Director/Co-director to support mobile health unit operations and outreach, including scheduling and deployment in rural communities of Cumberland County.
