Community Health Worker, The Freeman Center, Full Time, First Shift

The Community Health Worker will be to act as the liaison and advocate between UC Health patients and medical homes, community-based healthcare centers, or community providers to advocate for patients through educating, mentoring, coaching and outreach. The primary goal of the Community Healthcare Worker will be to identify patients who are in need of primary care, medical home needs, community resources and care coordination to promote basic healthcare access and meet social, physical and developmental needs for a diverse group of patient populations.
