Community Mental Health Practitioner

Job summary

We have an exciting opportunity for a qualified practitioner (Nurse/ OT/ Social Worker/ Mental Health Wellbeing Practitioner) to work within the Neighbourhood Mental Health Team in Heywood, Middleton and Rochdale. This is a 6‑month fixed‑term contract working across GP surgeries to support individuals aged 18 and over with severe and enduring mental health conditions, trauma and complex needs. The post holder will provide advice and triage, conduct clinical assessments where appropriate, offer short‑term interventions and facilitate referrals and transitions into suitable services within the transformed Community Mental Health Service. You will work as part of a highly supportive multidisciplinary team.
