Job Description
POSITION DESCRIPTION:
The Day Habilitation Case Manager is responsible for day to day case management of the individuals who are supported in the Day Habilitation Program on Roberts Drive. This person will manage a caseload of up to 15 people in CBDS. This position is responsible for: all tasks related to the development of DHSP’s in conjunction with Masshealth Regulations, including initial intake and assessment, DHSP development, 6 & 12 month progress reviews, annual general assessments, goal and objective development. Additionally, all Quality related consents and other documents must be updated annually. The Case Manager is required to communicate with family members, DDS workers and other departments as needed to ensure adequate services are being provided, and plans are implemented on a consistent regular basis.
