individual receiving service from IT’S MY LIFE, LLC CENTERED BASED DAY SUPPORT. Other responsibilities and duties include oversight of Individual Person-Centered Plans, PCP checklist and any behavioral Person Centered Plans. The PROGRAM MANAGER will also be responsible for the oversight of the admission, screening / assessment process and oversight of the CENTERED BASED & NON CENTERED BASED DAY SUPPORT Program.
