About the Role
We are seeking an experienced and compassionate Dementia Nurse Specialist to provide expert clinical leadership and person-centred care for people living with dementia and their whānau.
This role plays a key part in improving early identification, assessment, care planning and support across community and primary care settings.
You will work closely with individuals, families, primary care teams and community providers to deliver coordinated, culturally responsive dementia care, supporting quality of life and reducing avoidable hospitalisation.
This role can either be full-time or part-time and is fixed-term until June 2027.
