About the Role

We are currently looking for an experienced Families Together Practitioner to join our team. Key duties will include:

• Engaging families through strengths based and culturally safe relationship building
• Delivering intensive family support through home visits and community outreach
• Undertaking holistic strengths, needs, and risk assessments
• Developing and reviewing family led case plans and safety plans
• Providing parenting support through coaching, modelling, psychoeducation, and advocacy
