Job Description

Assist families in need as a Family Care Coordinator. Focus on developing and implementing customary care agreements to enhance support systems within Indigenous communities.
In this position, you'll work with the Customary Care Supervisor to coordinate meetings with various organizations and stakeholders. You're responsible for creating templates for agreements, reviewing compliance regularly, and processing caregiver payments. Additionally, you'll maintain secure records and support caregivers through wellness plans.
Key Responsibilities:
• Coordinate with First Nation and Indigenous agencies
• Create and implement Customary Care Agreements
• Conduct quarterly reviews for compliance adherence
• Process payments and manage financial documentation
• Facilitate communication with caregivers and families
Requirements:
• Preferred Social Work or related degree
• 2 years of related work experience
