We have been serving the community since 1979, across four primary care locations, and delivering care to over 23,000 patients. We are accredited by The Joint Commission, and our practice sites are recognized as Level 3 Patient-Centered Medical Homes by the National Committee for Quality Assurance (NCQA).

• Call is approximately 1:7; however, very light due to nurses taking call
