We have an exciting opportunity to join our team as a Field Nurse - Eastern Long Island- Home Care Division - SIGN ON BONUS - $5,000. 

In this role, the successful candidate Provides safe, quality, therapeutic age appropriate nursing care using evidence based practice. Uses clinical expertise in assessment, clinical decision making, clinical practice and patient education. The nurse coordinates the care provided with the interdisciplinary team to promote optimum patient outcomes and satisfaction.

Job Responsibilities:
