Become a First Link Care Navigator with the Alzheimer Society of Thunder Bay, focusing on responsive care for individuals facing dementia. This full-time position is dedicated to outreach and client support across various communities.
As the first point of contact for dementia-related conditions, you will assist clients and their care partners through challenging transitions. Your experience in therapeutic support will help facilitate meaningful connections with healthcare teams and streamline access to services. You will create and implement outreach initiatives to strengthen community engagement.
