Based out of Sarasota, FL and providing coverage to Sarasota, DeSoto, and Manatee counties., the Case Manager is responsible for coordinating and overseeing services to support individuals in the Sarasota Pilot Program with a primary focus on ensuring safety, stability, and assisting towards permanency.
