Job Description
Position Summary
The University Hospital is seeking a Geriatrician to provide inpatient and outpatient geriatric services at various sites. The successful applicant will become a member of the Department of Internal Medicine, but will not be required to participate in the call rota.
Scope of Services
- Assessment, treatment and follow-up in outpatient clinic or in hospital
- Medication review and education
- Referrals from family physicians or attending specialists for elderly patients living in the community or in a long‑term care facility
- Referrals from physicians with hospitalized acute care elderly patients whose medical, functional or psychosocial problems interfere with discharge or community function
- Complex competency assessments in collaboration with psychiatry
- Discharge planning
- Geriatric medical consultation and diagnosis
