About the position:
The Clinical Case Manager for Gilead’s Middletown area adult supervised apartment program works 40 hours, Sunday-Thursday from 7a-3p providing case management, advocacy, educational, and supportive services in a program serving women with major mental illness, co-occurring substance use disorders and histories of significant trauma. The individual in this role carries out all phases of clinical services including intake, engagement, assessment, treatment planning, case management, referral, and discharge. This role also requires the timely completion of required documentation including assessments and other treatment planning.
