JOB SUMMARY

Reporting to the Director of Stedman Community Hospice, and guided by SJLCB’s Mission, Vision and Values, the Grief and Bereavement Practitioner (GBP) operates as a member of the multidisciplinary team championing patient‑centred care in‑house and with the Outreach programs. The GBP will also assist in providing coverage throughout the Haldimand region. In collaboration with department colleagues, the GBP ensures prompt triage of referrals, completes assessments and makes appropriate referrals to hospice programmes and services.
