JOB DESCRIPTION
_The role typically begins with a training period, during which the employee works closely with a Care Coordinator within their assigned region. Upon completion of training, the employee is provided with an outreach list of 100 members._ **_Care Coordinators gradually build a full caseload of 45-50 members._**
**_Candidates are expected to have prior experience in care coordination and are required to reside in Lewis County._** _The position follows a hybrid model, with increased scheduling flexibility available after the first six months._ **_Job responsibilities include spending approximately 50% of the time in the field, conducting visits in members’ homes, healthcare facilities, or within the community._**
