Healthcare Social Worker — Senior & Community Care

ST LUKE'S HOSPITAL in Singapore is seeking a Social Worker to conduct psychosocial assessments and develop individualized care plans for patients transitioning from healthcare facilities to home. The role involves collaborating with multi-disciplinary teams and providing emotional support to patients and their families.

The ideal candidate will have a Degree in Social Work with 1 to 2 years of experience, preferably in healthcare, and must be able to work independently. Registration with Social Work Accreditation Board is preferred.
