Job Description
Responsibilities
- Provide case management support.
- Assess client's needs or presenting issues, where necessary provide timely interventions and actions for desired outcomes.
- Participate in case management meetings.
- Collaborate closely with various departments and agencies to meet the psycho-social emotional needs of clients to ensure a safe home or social environment for holistic care.
- Coordination of AIC or MSF or social referrals for financial or psychosocial care services.
- Support Medical Escort Transport team and Home Personal Care services for clients.
- Application for financial grants, FA case report and financial counselling.
- Provide home environment safety, hygiene, well-being supportive care and interventions.
- Support community outreach.
- Assist in social documentation.
