Home-Based Community Mental Health Social Worker

ST LUKE'S HOSPITAL in Singapore is seeking a Medical Social Worker to conduct mental health assessments and provide direct interventions to clients with mental health issues and dementia.

The ideal candidate will have a degree in Social Work and at least 2-3 years of experience in community or hospital settings. Responsibilities include maintaining accurate client records and supporting service planning in accordance with the COMIT programme.
