Position Overview
The RN Case Manager is responsible for coordinating and managing patient care in the home health setting. This role focuses on comprehensive case management, ensuring high quality, patient centered care while meeting productivity and documentation standards.
Schedule & Work Details
- Coverage Areas: Nash, Edgecombe, and Wilson Counties
- Schedule: Monday through Friday, 8:00 AM to 4:30 PM
- Weekly Hour Guarantee: 36 hours
- On Call/Weekend Requirement: No
- Mileage Reimbursement: Yes, per contract company policy
- Dress Code: Scrubs, no specific color required
- Pay Rate: $51 to $53 per hour
