- Schedule: PRN, successful candidates will have 2-3 days of availability weekly from 7:00a-4:00p

- Reports to: Branch Director
- Assess each new patient's condition, history, and home care needs
- Build the care plan with the patient and family
- Update the care plan as the patient's status changes
- Administer medications and treatments as prescribed
- Teach patients and families how to manage care at home
- Coordinate with physicians and send updates at least every 60 days
