Intake Coordinator RN - PRN Edinburg

POSITION SUMMARY:

The Rehab Liaison receives patient referrals via phone or fax.  He/she arranges for evaluations and reviews initial payor data.  This individual secures appropriate contact information and assesses patient’s rehabilitation potential and suitability for hospitalization.  They interview patient and family to gather information and determine special needs.  They must adhere to admission criteria and present potential patients for approval at admissions meeting.  They also inform physicians of pending admission and coordinate pertinent data with Business and Admitting Office.  They represent the facility in all contracts with other health professionals and the general public in a manner which enhances the facility’s reputation.  This position summary contains the most relevant duties and responsibilities and does not exclude other assignments not me...
