Coverage Needed

• Start Date: 10/19/2026
• End Date: 4/30/2027
• Schedule: 5 x 8-hour shifts starting at 8a and 11a (1-2 evening shifts/week)
  • Some flexibility for 12-hour shifts depending on assignment
  • No option for 4 10s
  • 4 weeks/month preferred, but a couple weeks/month and part timers are also acceptable
• Home base between 1-2 locations to keep provider in a reasonable commutable area

• Practice Settings: Clinic
• Patients Per Day: 18
• Support Staff: Robust support staff including Medical Assistants, RNs, LVNs, Behavioral Health on-site, and multiple resources at each site
