Lead Health and Social Care Support Worker

Overview

An exciting opportunity has arisen for a band 4 Lead Senior Health & Social Care Worker to join the Discharge to Assess Service Nairn & Ardersier on a permanent basis. You will be expected to work flexibly to meet the needs of the service which will mean rotation of shifts to ensure service provision over 7 days.

Responsibilities

• Support the caseload holder to deliver and assess patient-centred care.
• Work in close partnership with colleagues, ensuring an integrated approach to the delivery of compassionate care.
• Work across complex multi-agency environments with partnership working as essential, requiring high levels of professionalism and excellent communication skills.

Qualifications/Requirements

• SSSC registration.
• Driving licence with access to a car.
• Ability to work flexibly and rotate shifts to cover 7-day service provision.
