Licensed Therapist / Occupational Therapist Job in Chicago, Illinois / Home Health Care

We are seeking an Occupational Therapist (OT) for a full-time contract opportunity for a home care position in Chicago, IL.
The Occupational Therapist plans and conducts individualized occupational therapy programs in a home care environment to help patients develop, regain, or maintain their ability to perform daily activities. He or she teaches patients skills/techniques and how to use adaptive equipment for participating in activities.
The OT will also study, evaluate, and record patients' activities and progress. He or she may also:
-Evaluate a patient's condition and needs
-Develop a treatment plan for patients, identifying specific goals and the types of activities that will be used to help the patient work toward those goals
-Educate a patient's family and employer about how to accommodate and care for the patient
