Licensed Therapist / Physical Therapist Job in Nashville, Tennessee / Home Health Care

We are seeking a Physical Therapist (PT) for a full-time contract opportunity for a home care position in Nashville, TN.
The Physical Therapist is responsible for evaluating/assessing needs of referred patients and formulating treatment plans in a home care environment.
Responsibilities included but not limited to are:
-Provides therapy services defined in treatment plans
-Meets the patient's goals and needs and provides quality care by assessing and interpreting evaluations and test results; determining physical therapy treatment plans in consultation with physicians or by prescription.
-Work cooperatively with physicians, case managers, and adjusters
-Evaluates effects of physical therapy treatments and fit of prosthetic and orthotic devices by observing, noting, and evaluating patient's progress; recommending adjustments and modifications.
