Catholic Health Home Care is looking for passionate, dedicated LPNs to provide comprehensive care to patients at home.
Team 5: Bohemia, Brentwood, Fire Island, Great River, Islandia, Islip , Islip Terrace, Lake Ronkonkoma, Oakdale, & West Sayville.
+ 5x 8 hour shifts (8:30AM to 4:30PM) - Rotating Weekends
+ Great Healthcare benefits, and education reimbursement of $5,250 annually
+ Additional compensation for visits over your caseload
Job Details
Our Home Care LPN’s:
+ Case manage, assess, plan, develop and implement a comprehensive home health care plan for their patients.
+ Measure vital signs, Administer IVs, change dressings, clean wounds as needed
+ Provides skilled nursing care, preventative rehabilitative procedures, and prescribed treatments with a variety of patient populations within various potentially complex h...
