**The Manager Hospital Care Coordination:**

+ Provides overall management of the department team members.
+ Monitors length of stay and identifies way to reduce length of stay through case management and identification of safe alternatives to inpatient care.
+ Promotes and supports BayCare strategic directions and goals (including but not limited to reducing hospital readmission, observation length of stay, patient experience, and team member as customer).
