The Medical Social Workers are integral to this model, providing psychosocial and financial assessments, casework, and interventions. They also collaborate with the multi-disciplinary team of stakeholders to review individual residents' medical, nursing, therapy, and social aspects as part of delivering person-centric care. This support is crucial throughout the residents' journey, from their admission to their entire stay in the Home.
