Job Responsibilities- Provide bio‑psychosocial assessment and intervention to support patients / residents / clients / caregivers in accordance with professional standards of practice
- Deliver end‑to‑end social work services, including case assessment, care planning, intervention, evaluation, counselling, and emotional support
- Assist in addressing psychosocial, financial, and discharge planning needs as part of overall care management
