This role supports Veterans in a multidisciplinary outpatient environment by combining clinical expertise, care coordination, patient education, and procedural assistance. You will work closely with physicians and other healthcare professionals to support patient-centered care, focusing on wellness and disease management. Your expertise will be essential in assessing patient needs and coordinating healthcare services to ensure optimal patient outcomes.
